Cardiac sarcoidosis: Difficulties and possibilities of differential diagnosis for acute coronary syndrome without ST segment elevation in real clinical practice

چکیده
Here we present a clinical case of cardiac sarcoidosis. The article discusses the difficulties differential diagnosis in patients emergently hospitalized cardiology department with recurrent chest pain, no changes electrocardiogram (ECG), and presumptive acute coronary syndrome without ST segment elevation (NSTE-ACS). A careful history taking, invasive angiography, contrast-enhanced (gadolinium-based contrast media) magnetic resonance imaging (CMR) contributed to correct diagnostic decision search for identification sarcoidosis patient.

OBJECTIVE To evaluate different characteristics of patients with acute coronary syndrome (ACS) without ST-segment elevation compared with transient St-segment elevation. DESIGN An observational, prospective study. SETTING A 12-bed coronary care unit. PATIENTS Consecutive patients of ACS without persistent ST-segment elevation. AIMS We sought to determine whether the extent of myocardial ischaemia on the admission electrocardiogram (ECG) has independent predictive value for short-term risk stratification of patients with non-ST-segment elevation acute coronary syndromes (NSTE ACS). Although the presence of ischaemic ECG changes on admission has been shown to predict outcome, the relationship between the extent of ECG ...
متن کاملBleeding in non-ST-segment elevation acute coronary syndrome.
The development of antiplatelet and antithrombotic therapies, in addition to interventionist strategy, has resulted in great improvements in the outcomes of patients with non-ST-segment elevation acute coronary syndrome. Parallel to therapeutic advances, bleeding, which can be induced during management, increases the risk of recurrent ischemia, myocardial infarction and death. The present liter...

m n s i r cute coronary syndromes (ACS) account for approxiately 1.6 million annual inpatient hospital discharges in he U.S. (1). Approximately 80% of these are due to on–ST-segment elevations ACS (NSTE-ACS). In this ear’s review, we highlight publications and presentations etween May 2006 and April 2007 in 6 areas: risk assessent, antithrombotic therapy, anti-ischemic therapy, perutaneous coro...

Patients admitted to hospital with symptoms and signs of non-ST-segment elevation acute coronary syndromes have different risk profiles and are in need of an individualized approach that takes into consideration not only age and sex but also comorbidities such as diabetes, renal failure, hypertension, heart failure, peripheral artery disease, earlier revascularization, etc. According to evidenc...
